Instructions:

1. Choose to Continue to online forms

2. Sign in, create an account, or continue as guest

3. Make sure all Names are correct and make sure your School I.D. # is entered correctly (Example: Do not enter your student I.D. number as 001111, just enter 1111.)

4. Make sure that you fill out and sign all forms

Completed physical exam forms will be uploaded by Athletic Trainers. A new physical exam will be required each school year.
